  Please tell us about yourself, your playing and why music is important to you. What you would like to achieve and how a better instrument or bow might help you to achieve it? What types of music do you enjoy playing? What music do you take part in other than your lessons such as orchestras, ensembles and groups. Do you play in concerts or festivals, and what do you enjoy most about playing your instrument?

A Teacher's Statement is also required. We will request this via email once this form has been approved.
